Attention CMS Parents! GraceMed School Based Dental Health forms went home with your child today! To comply with Kansas State Statute 72-6251, all children are offered annual school-based dental screenings. Circle USD 375 utilizes the services of GraceMed Health Clinic for dental screenings at school. If you wish for your child to obtain Extended Care Services (cleaning, fluoride, sealants), please complete the consent portion of the form. If you wish for your child to receive a screening only (no cleaning, fluoride or sealants), no action is required. For those wishing to opt their child out of ALL dental services, including screening, provided at school, simply write the words "opt-out" on your child's form and return it to the school office.
