Ages & Stages Questionnaires

PreKindergarten & Kindergarten Developmental Screening

Your child’s first five years are an important time for learning and growth, and we’re here to support you every step of the way. As part of that support, we ask that you complete the Ages & Stages Questionnaires (ASQ-3 and ASQ:SE-2) to help track your child’s development over time.

These questionnaires may be completed every 2, 4, or 6 months and include simple questions about what your child can do in areas such as communication, movement, problem solving, and social skills.

The information you share is secure and only accessible to the program staff who invited you to participate. 

Families might receive specific instructions from their child’s school on when to complete the ASQ, but for the most accurate and timely results, ASQ Stages 2 and 3 should be completed after April 1.

Complete ASQ 2 & 3

To complete the survey:

  1. Click the link below for your school.
  2. Select “I am completing both the ASQ-3 and ASQ:SE-2.”
  3. Enter your child’s birth date.
